Insurance

• We are now a Blue Shield PPO provider (effective Feb. 12, 2015)
• We are contracted with MOST PPO providers. We are NOT contracted with Pacificare or Aetna. This means we can still bill your insurance companies, however the benefits are out-of-network. We suggest you call your insurance company and ask for your benefits.
• If you are unsure of the coverage by your providers, please contact us and we will assist you in determining your deductibles and allowable coverages.
• Workers' Compensation is accepted only upon prior authorization
• Medicare is accepted.
• We are not contracted with MediCal. Allowable coverages must be verified prior to your visit.
